- ✪ ✪ ✪ ✪ ✪This Doctor is unprofessional. I had doctor ordered work release after being hospitalized for an illness. I've been trying to have the doctor fill out the doctor's certification form for my SDI benefits to process. It's been nearly three weeks and she has not filled out the form. I asked if they could do the form online (like most doctors do these days), quick and easy, but they said "we're not set up for that". Really?!?!? It's 2017, EVERYTHING IS ELECTRONIC! Now because of this doctor, I have no idea how I'm going to pay my rent and keep my electricity on with 5 children in my home.September 13, 2017by Jason Morris
